• ​Name and contact information for the passenger and/or care-giver
• Name of the companion/care-giver if accompanying the passenger
• Date, day and time requested for pick-up
• Medical/other appointment time if applicable
• Exact address for the pick-up
• Exact address for the destination
• Special needs if applicable: walker, wheelchair, etc. Individuals in wheelchairs should be accompanied by an aide.
• Return trip information if needed
